  • Overview

    We are seeking a medical assistant (MA) or licensed practical nurse (LPN) to join an innovative team that is committed to improving care for underserved patients with complex-needs. Reporting to the Field Team Physician, the Field Team Coordinator (Clinical) will be based in our office in Detroit but will include travel throughout the local area.

     

    The Concerto Health Field Team is a robust, multi-disciplinary team that provides wrap-around services to patients in the home and community setting. Their aim is partner with provider groups to ensure effective and safe transitions of care, improve quality of care, and in doing so prevent unnecessary utilization (emergency department visits, inpatient admissions, readmissions).

    Responsibilities

    The Clinical Field Team Coordinator (FTC) provides administrative support to the Field Team and works closely with the provider team to provide clinical support during home visits. The Clinical FTC is responsible for managing the home visit schedule, stocking field team supplies and supporting inventory and ordering, coordinating referrals, home infusion, and diagnostic services, requesting records, utilizing tasking guidelines to route inbound calls, and supporting seamless coordination and communication between the Field Team, patient and caregiver, Primary Care Team, and Care Management Team.

     

    The Clinical FTC performs clinical work under the supervision of the provider and within state defined scope of practice, to include patient intake, health screenings, medication and immunization administration, dressing changes, and additional duties as assigned to support patient care needs. The Clinical FTC employs a propriety population health platform to close preventive care gaps per standing order, perform medication reconciliation, and support seamless patient care coordination.

     

    The Clinical FTC priorities workflow and implements Concerto tasking guidelines to effectively communicate both in-person and electronically with team members and ensure patients and team members receive timely response to questions, requests, and concerns.

     

    The Clinical FTC performs all duties in a manner that reflects that mission, vision, and core values of the Concerto Health model. The Clinical FTC is accountable for delivery of quality, patient-centered, and team-based care and adherence to Concerto clinical policies, procedures and guidelines. He or she demonstrates dedication to and an aptitude for innovation, quality improvement, role optimization, and team-based care. This individual demonstrates versatility, flexibility, and a willingness to work with constantly changing priorities with enthusiasm and a positive attitude and promote this same attitude among team members. Performs other duties as assigned.

    Qualifications

    • Medical Assistant or Licensed Practice Nurse
    • High School Diploma or equivalent
    • Valid driver’s license and car insurance
    • Ideally three or more years’ experience since completion of residency, including home visit experience
    • Experience providing care for the whole person, including patients living with mental illness and substance use disorders.
    • Strong interpersonal and collaborative problem-solving skills
    • Ideal candidates will want to work as advocates for their patients. They will also embrace the concept of team-work and mutual support of their colleagues, fostering a joyful work culture.
    • Current CPR and BLS certification preferred
    • Average to advanced computer & software skills (Microsoft Word, Excel, Outlook)
    • Ability to work a flexible schedule, including periodic outreach duties evenings, nights, and weekends.
    • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving with 7 years.
