We are seeking a primary care physician leader to join our innovative team in Chicago, Illinois that is committed to improving care for under-served and complex-needs patients. Reporting to the Chief Medical Officer, the Medical Director will be based in our office in Chicago, Illinois.
The Medical Director must have a strong desire to improve health outcomes for our members. The Medical Director will provide oversight to a field-based provider team of physicians and advanced practitioners, as well as provide clinical direction to education to other team members, including nurse care managers, social workers, and patient care coordinators. He or she will build close relationships with members of the care management team, and be committed to working in a collaborative fashion to provide excellent care for patients. They must possess excellent time management and organizational skills, with the ability to prioritize and multi-task. Flexible and dynamic, a creative problem solver, this self-starting candidate will exhibit a high level of professionalism that allows them to lead and work effectively with a team of dynamic and unique individuals.
Specific duties include leadership of field-based provider team and participation in intradisciplinary care team meetings to develop care plans for members. The Medical Director is expected to provide clinical direction and education to other team members. Other responsibilities include meeting with network providers regarding care coordination, documentation initiatives, and quality programs.
