The Manager, Long Term Supports and Services (LTSS) is responsible for the overall management of a team of Social Workers and LTSS support staff who work with individuals with complex health and/or behavioral needs. The LTSS team coordinates personal care and Waiver services for eligible individuals through clinical assessment and ongoing monitoring of needs. The Manager, LTSS will be responsible for revenue driving activities related to LTSS/Waiver, completion of Personal Care Assessments, Nursing Facility Level of Care (NFLOC) documents, and authorization processing for all LTSS services. The Manager, LTSS will possess knowledge of and remain updated on client program requirements and will provide ongoing education/training to Case Management/LTSS staff.


  • Ensure that required documentation for waivers services and Nursing Facility Level of Care documentation is completed and submitted to the state in a timely manner.
  • Conduct consistent performance management of all social services staff, manage and performs data analysis and reporting against defined metrics or other requirements, as well as prepare and respond to audit or other external reporting. 
  • Support and promote an on-going person-centered planning process.
  • Direct supervision of LTSS Supervisor and LTSS Supports Coordinators (Social Workers)
  • Oversee the authorization process for all LTSS services with working knowledge of Medicaid covered services.
  • Participate in outreach activities including representing Concerto health at community events as required.
  • Review clinical documentation, assess employee performance and productivity, and provide feedback to staff for ongoing quality improvement.
  • Conduct regularly scheduled team meetings and facilitate clinical discussion among team members.
  • Participate in company staff meetings and training activities.
  • Provide mentoring, training, and support to LTSS Supports Coordinator and LTSS Coordinators. Serve as a subject matter expert on LTSS requirements.
  • Assist in identifying needs for process improvement and communicate this to leadership.
  • Serve as a member of the leadership team and lead by example.
  • Collaborate with other departments to meet company goals.
  • Participate in interviewing potential employees.
  • Clinical supervision for licensure of Limited Licensed Social Work employees.
  • Participate in collaboration with PIHP providers to ensure integration of medical, LTSS and mental health services.
  • Attend state and client meetings.
  • Works as a member of the leadership team and supports company initiatives and keeps Director of Care Management appraised of departmental issues.


The following licenses meet required qualification for this position:

  • Licensed Masters in Social Work with full state licensure.
  • At least three years of experience in a healthcare environment.
  • Experience with populations similar to the Medicare/Medicaid Enrollee population, including community-based and facility-based individuals with medical, LTSS and/or behavioral needs.

Competencies Required:

  • Problem-solving and organizational abilities, as well as the ability to work harmoniously, constructively and cooperatively with other professionals, members and their families, community service providers, members of the general public, etc.
  • Proficient with Microsoft Office software.
  • Capacity to work independently or interdependently and provide leadership to a team.
