The Manager, Long Term Supports and Services (LTSS) is responsible for the overall management of a team of Social Workers and LTSS support staff who work with individuals with complex health and/or behavioral needs. The LTSS team coordinates personal care and Waiver services for eligible individuals through clinical assessment and ongoing monitoring of needs. The Manager, LTSS will be responsible for revenue driving activities related to LTSS/Waiver, completion of Personal Care Assessments, Nursing Facility Level of Care (NFLOC) documents, and authorization processing for all LTSS services. The Manager, LTSS will possess knowledge of and remain updated on client program requirements and will provide ongoing education/training to Case Management/LTSS staff.
