We currently operate in 2 markets (Michigan and Washington State) that include 7 primary care practices, and do so in partnership with innovative health plans that share our vision.
Our Behavioral Health Specialist will serve as a key member of the Patient Engagement Team, addressing the needs of our most medically and socially complex patients. The Patient Engagement Team is a developing initiative to meet patients “where they are at,” build supportive relationships which will address their social needs, and ensure better engagement in their medical and behavioral health care.
The Behavioral Health Specialist also serves as a part of the integrated primary care practice team, alongside primary care providers and other core team members. Through a combination of individual and group therapies, community resource connection and creative integration with our primary care offerings, the Behavioral Health Specialist will guide patients to behavioral and physical health.
The Behavioral Health Specialist will serve as a behavioral health extender for the rest of the team, increasing team members' understanding of behavioral health assessment and treatment.  The Behavioral Health Specialist will work closely with the Medical Director, RN Practice Manager, Clinic Operations Supervisor, and Social Services Supervisor.


  • Specific responsibilities will include, but are not limited to, the following:
  • Participating in outreach and engagement strategies with a select group of high-utilizers and at-risk patients
  • Providing brief, focused individual therapy in collaboration with a multi-disciplinary team
  • Evaluating crisis situations and employing appropriate interventions
  • Improving population health outcomes by ensuring all patients and primary care team members have access to behavioral health expertise
  • Facilitating and overseeing referrals to specialty mental health services and supporting a smooth transition from primary care to specialty mental health services
  • Facilitating group-based visits and workshops to holistically address medical and behavioral health needs
  • Providing education and consultation to providers and other team members on the basic principles of brief behavioral and cognitive behavioral interventions and reinforcing their use in medical visits
  • Creating new ways to engage a population in behavioral health beyond traditional venues
  • Attend scheduled multi-disciplinary team meetings and provide relevant recommendations to improve the health of high risk patients and those who have recently been hospitalized
  • Travel between care center locations may be required (Lynnwood, Renton, and Lakewood Care Centers)
  • Performing other duties as assigned.


  • Must be Licensed Independent Clinical Social Worker, or Licensed Mental Health Counselor, licensed to practice independently in the State of Washington
  • LICSW and LMHC.  Licensed Independent Clinical Social Worker or Licensed Mental Health Counselor. 
  • Must have a master’s degree or doctoral degree in Social Work, Counseling or other behavioral health field
  • Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions
  • Competency in cognitive behavioral therapy and motivational interviewing preferred
  • Experience working in integrated medical settings preferred
  • Exceptional capacity to multi-task
  • Must be flexible and comfortable with ambiguity
  • Excited to think about addressing behavioral health issues in a new way and in a team setting
  • Experience and/or ability to work with electronic health record systems
  • Average to advanced computer & software skills (Microsoft Word, Excel, Outlook)
  • Excellent communication and customer service skills
  • Ability and means to travel as needed in a timely manner within King, Pierce and Snohomish Counties, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving with 7 years.
